Student's commitment to the practicum experience: Consistently Committed (5 Points)
(1) Comments:
Emily has shown an above average level of commitment to her practicum experience and has taken a great deal of initiative to go above and beyond anything I’ve asked her to do for the campaign.
Student's work habits to date: Excellent (5 Points)
(2) Comments:
Emily’s work habits are indicative of a student that is focused and task oriented.  Due to the amount of task she has taken responsibility of, I am extremely impressed with the turnaround time in which Emily has completed or is working toward completion on each of these vital pieces of the End the Wait Campaign.
Overall quality of student's effort (e.g., amount learned, skills learned, etc.): Very Satisfied (5 Points)
(3) Comments:
I honestly could noy be any more satisfied with Emily's effort.
Progress toward practicum product(s)/project from the student: Very Satisfied (15 Points)
(4) Comments:
Emily’s practicum is awith  long-term statewide issue campaign surrounding the Developmental Disabilities Waiting List and the work we are doing now is critical to the overall outcome of the campaign.  Emily is progressing wonderfully toward the goals we set out in the beginning.
Student's interaction with others at the site (e.g. staff, clients): Consistently Positive (5 Points)
(5) Comments:
The site for Emily’s practicum is within the Disability Rights Center of Kansas office in Topeka and we are somewhat tucked away in the back of the office.  With that said, we don’t have a great deal of contact with the DRC staff.  We will, however, have ample opportunity to interact with the public and members of the Kansas Legislature in the future.  Anytime Emily has interacted with DRC staff she has done so with a great attitude and exhibited professionalism.
Student's relationship with others in the organization: (e.g.  effective working relationships within the organization and the community.) Very Satisfied (5 Points)
(6) Comments:
Emily has excellent relationship skills and works very effectively with others.
Student's overall attitude toward the activities and personnel associated with the practicum: Very Positive (5 Points)
(7) Comments:
Emily approaches her work with enthusiasm and I am pleased with how she interacts with me and others involved with the campaign.
Student's understanding of practicum objectives: Very Good (5 Points)
(8) Comments:

Student's level of dependability to date: Consistently Dependable (4 points)
