Midterm Practicum Student Evaluation (For SUPERVISORS)

Student's commitment to the practicum experience: Usually Committed (3 Points)
(1) Comments:
It appears that Alisa is fully committed to this project - though scheduling and 'start-up' have taken a bit longer than anticipated.
Student's work habits to date: Very Good (4 Points)
(2) Comments:
Alisa seems to demonstrate good work habits in our conversations about her 'plan'
Overall quality of student's effort (e.g., amount learned, skills learned, etc.): Satisfied (4 Points)
(3) Comments:
Again, she is at beginning of starting up her project so I can't fully answer this question for mid-term eval. BUT Alisa definitely seems hard-working and dedicated to seeing it through.
Progress toward practicum product(s)/project from the student: Somewhat Dissatisfied (9 Points)
(4) Comments:
I only mark this response because I believe Alisa would like to have been further along with her project & I know the agency has worried about her getting what she needs from us to make that happen. I will plan to set up standing mtgs with her for remainder of the placement to ensure her needs, and those of the practicum, are being met.
Student's interaction with others at the site (e.g. staff, clients): Consistently Positive (5 Points)

I have never seen Alisa being anything but positive!
Student's relationship with others in the organization: (e.g.  effective working relationships within the organization and the community.) Very Satisfied (5 Points)

What I have seen - Alisa seems to interact with staff members very well.
Student's overall attitude toward the activities and personnel associated with the practicum: Very Positive (5 Points)

Again, she is always positive when I have seen and/or met with her.
Student's understanding of practicum objectives: Very Good (5 Points)

Alisa seemed very knowledgable of her practicum objectives when she initially met with Becca and I.
Student's level of dependability to date: Almost Always Dependable (3 points)

I know there has been one incident of scheduling conflicts with her other job - but I believe she was working that out with them.
